Survey on hallucination classification and mitigation in medical large vision-language models

Abstract

Medical Large Vision-Language Models (Med-LVLMs) commonly exhibit hallucination, i. e. , erroneous perception and reasoning about medical images, which severely hinders their application in real clinical scenarios. Since existing studies mainly classify hallucinations by their superficial manifestations but rarely analyze the underlying formation mechanism, this review classified hallucinations into two categories from the perspective of this mechanism: visual factual inconsistency and cross-modal reasoning inconsistency. On this basis, the review systematically analyzed the underlying causes of hallucinations across multiple dimensions, including perception errors of the visual encoder, semantic mapping deviation of the cross-modal connector, the visual detachment effect during large language model reasoning, and the absence of structured medical logic. Based on this causal framework, the review comprehensively examined existing hallucination mitigation strategies and evaluation systems, aiming to mitigate hallucinations in a more targeted manner. Finally, the review outlined future research trends in hallucination mitigation for Med-LVLMs.
